Diabetes and Aging: A Very Common Combination

India has one of the highest rates of diabetes in the world — and the prevalence increases significantly with age. In seniors above 60, diabetes affects roughly 1 in 4 individuals. Managing diabetes in an older adult is not simply a younger person's diabetes management plan adjusted for age — it is meaningfully different, and those differences matter enormously.

Why Diabetes Management Is More Complex in Seniors

•       Hypoglycaemia (low blood sugar) is more dangerous — seniors may not feel warning symptoms until critically low

•       Cognitive impairment affects ability to self-manage medications and diet

•       Multiple other conditions (hypertension, kidney disease, heart disease) complicate treatment

•       Reduced appetite and weight loss can change insulin and medication requirements

•       Foot complications — neuropathy (nerve damage) and poor circulation — develop more quickly

•       Side effects of diabetes medications are more pronounced in older bodies

The Dangers of Hypoglycaemia in Elderly Diabetics

Hypoglycaemia — blood sugar dropping too low — is the most immediately dangerous complication for elderly diabetics. Unlike younger patients who feel shaky, sweaty, and confused, many seniors have 'hypoglycaemia unawareness' — they show no early symptoms and can lapse into severe confusion or unconsciousness without warning.

This is why blood sugar targets for elderly diabetics are often set slightly higher than for younger patients, and why overcorrection with aggressive medication is genuinely dangerous.

Signs to watch for in an elderly diabetic: sudden unusual confusion, unusual fatigue, pale or sweaty skin, unresponsiveness. If in doubt — check blood sugar immediately.

Foot Care: A Non-Negotiable Priority

Diabetic neuropathy reduces sensation in the feet. This means a blister, a small cut, or a pressure sore on the foot may go completely unfelt — while silently progressing to a serious infection, or in the worst cases, requiring amputation.

Daily foot care is essential:

•       Inspect both feet every single day — use a mirror for the soles if needed

•       Wash feet gently with lukewarm water and dry completely between toes

•       Moisturise the feet but not between toes

•       Never walk barefoot — always wear clean, well-fitting footwear

•       Trim nails straight across, never curved — or have a nurse or podiatrist do it

•       Any wound, blister, or discolouration on the foot requires medical review within 24 hours

Dietary Management for Elderly Diabetics

South Indian food, while nutritious in many ways, can be high in refined carbohydrates (white rice, maida, white bread) that spike blood sugar. Practical adjustments:

•       Replace white rice with red rice, brown rice, or mix 50/50

•       Increase vegetables and lentils in every meal

•       Reduce portion sizes of carbohydrate-heavy foods

•       Choose idli and dosa over fried snacks — but monitor quantity

•       Eat at consistent times — irregular meals destabilise blood sugar

•       Limit fruit juices and sweet drinks entirely

Monitoring and Medication

Blood sugar monitoring frequency should be determined by the treating physician based on the individual's condition and medication type. For insulin-dependent elderly patients, family members should be trained to check blood sugar and recognise hypoglycaemia.

Medications must be reviewed regularly as the senior's weight, kidney function, and other conditions change. Never stop or adjust diabetes medication without medical guidance.
